Ticker Symbol

The ticker symbol is a system of letters used to uniquely identify a stock. The number of letters in the symbol will depend on the exchange the stock is listed.

Example: Computershare = CPU

Account Number

Enter the Account Number printed on a statement or on your security certificates. It will be 10 or 11 digits long and may begin with a letter. Examples: C0000012345 or 0000012345.

Zipcode

Enter all numbers as listed on your statement or certificate. Please enter the first 5 numbers of your Z ip Code.

Famliy or Company Name

Enter your surname as it appears on your statement or certificate. If you have a suffix in your name, such as "Sr" or "III" do not enter it. Example: For "John E. Doe III" enter "Doe".

If the Securityholding is held jointly, please enter the surname (or full company name) of the first registered holder. Example: For "John E. Doe III + Jane Doe JT" enter "Doe"

If the Securityholding is held in a company name, please enter the full name of the company exactly as it appears on your statement/certificate/dividend advice. Example: For "Test Company Inc" enter "Test Company Inc"
